* fix: Handle GitLab Flavored Markdown

* PR feedback

* Harden HTML comment stripping against overlapping patterns

Loop the replacement until stable to avoid CodeQL's incomplete
multi-character sanitization alert — a single pass could leave
`<!--` residue for inputs like `<!<!-- x -->-- -->`.
